Pre-Registered/Returning Students - Step 2

Step 2 of 3- Complete all district forms

The MVUSD website contains several forms that must be completed and returned during registration. All forms can be downloaded from the district website or picked up at the district office during operating hours.
  • Click on Returning Student Registration page on the MVUSD website.
  • Read the "Read Only" forms.
  • Fill-in online, download, and print the required forms 

If you have a change of address, please submit a copy of a current utility bill with the Emergency Release/Health form before the address can be changed.
