MVUSD Online Course Contract  (Driver's Education Form)

*All forms need to be printed off and taken to your home school counselor to set up with MCA for Independent PE or Online Class scheduling. After submitting the form, you will have the opportunity to print the form. 

 *Both forms need to be completed to be considered for online PE.  If professional sports are the reason for participating in online P.E., please provide a letter from a professional coach (i.e. Junior Olympic Coach

*Drivers Education- Please fill out the MVUSD Online Course Contract and select Drivers Education. Students must contact the resident school academic counselor to submit the Drivers' Education Online form.
