    **IMPORTANT VACCINE NOTICE**
     
     
    Whooping Cough/Pertussis (Tdap) Vaccine required for School Entry in 2021-2022
     
     
    • New legislation REQUIRES that all students entering grades 7th thru 12th grade must receive a Tdap booster immunization prior to next year’s school enrollment.
    • Failure to meet the requirement will result in your student being unable to enroll in classes for the new school year.
    • Please send/bring your student’s proof of the Pertussis booster immunization to the health office for documentation.
    • You can check your immunization records for the following “DTap/Tdap/DTP” on or after their 7th birthday (Td does not meet the requirement) or feel free to contact the school health office to verify most recent date (696-1406 ext 4093)
    • Please note this immunization is in addition to other DTP, Dtap, or Td immunizations
    • Consult your health care provider for further information or contact the Riverside County Family Care Center: Lake Elsinore Family Care Center, 2499 E. Lakeshore Drive, Lake Elsinore, CA. 92530. Monday – Friday 8:00-4:00 pm.   Please call 1-800-720-9553  for further information.

    Shivela's health office is staffed by School Nurse Danielle Hunkle, BSN, R.N. and Health Technician, Dena Lopez. Nurse Hunkle also serves students at Tovashal. The Shivela Health Office staff provides general first aid and emergency care to students and staff. Medication is dispensed to students with a Physician's Authorization on file. Students that require specialized health care may also receive assistance in the Health Office. Vision and hearing screenings are conducted as well as other health screenings and assessments as needed. We are here to promote health and healthy lifestyles for our students here at Shivela.
