This consent form allows for the release of student records to the California College Guidance Initiative which helps prepare students grades 6 -12 for college and career readiness upon graduation from high school.

In order to properly care for students and notify parents when a student is ill or injured, all parents are required to submit an emergency card with phone numbers, emergency contacts &  information about any health condition their child may have. 

The Murrieta Valley Unified School District, in cooperation with the California Departments of Health Care Services and Education, participates in a program that allows the district to be reimbursed with federal Medicaid dollars for select health services provided to enrolled Medi-Cal students at school. The money received through this program is directly reinvested into expanding and improving health and social services for all students.

The school MUST have written parent and physician permission on file, updated annually, before a student can take medication at school.  All medications must be kept in the health office except asthma inhalers, epi-pens & diabetic supplies can be carried with parent and physician's request.
