Proof of Residency
Please submit two documents from the list below.
AT LEAST ONE DOCUMENT MUST COME FROM COLUMN A.
Both documents must show your name, current physical address and be dated within 30 days.
(Must choose at least 1)
❏ Utility bill, both top AND bottom showing same physical address AND dated within the last 30 days
❏ Mortgage Statement
❏ Homeowners/Renters Insurance
WE DO NOT ACCEPT GRANT DEEDS
WE DO NOT ACCEPT UTILITY BILLS WITH DISCONNECTION WARNINGS/NOTICES.
❏ Form or letter from a state or federal government agency received within the last 30 days
(Examples: Government assistance, jury duty summons, or change of address request from post office)
❏ Current car insurance policy declarations page, summary or Proof of Insurance Card with current address listed
❏ Cell Phone Bill
❏ Bank Statement
❏ Current vehicle registration card from the DMV (We do not accept renewals.)
❏ Payroll check/stub dated within the last 30 days.

❏ Edison or Electric Bill
❏ Propane Bill
If you have recently set up service, please contact Edison at
990-7788 to request a Residency Letter, which will be emailed to you

If the student is living with another family within district boundaries, you must complete a Caregiver affidavit before enrollment can be completed. (Note: This affidavit is subject to verification by a home visit or other means.)
If you are unable to produce proof of residency because the student is in a homeless situation, please call the District Homeless Liaison Yoana Armendariz at the District Support Center, (951) 696-1600, extension 1216 or email Yoana for assistance.
