Preschool/Child Care Immunization Requirements (18 months - 5 years)

The following immunizations are required for school enrollment by the State of California. An official record of the student's immunization record must be presented as verification of immunization. This record can be a California Yellow Immunization Card or a medical record from the physician. The record must be signed or stamped by the physician. Polio 3 doses at any age Diphtheria, Pertussis, and Tetanus (DPT) 4 doses at any ageMeasles, Mumps, and Rubella (MMR)Must have been given on or after 1st birthday* 1 dose
Hib 1 doseVaricella ChickenpoxKindergarten and all grades 1-12 students enrolling in a California school for the first timeA four (4) day grace period is allowed if vaccine was given 4 days prior to birthday.1 dose ora physician-documented disease history
All immunizations must be current, meaning the student has completed the above requirements or are not yet due for another dose.
