Preschool/Child Care Immunization Requirements (18 months - 5 years)
State of California Required Immunizations
The following immunizations are required for school enrollment by the State of California. An official record of the student's immunization record must be presented as verification of immunization. This record can be a California Yellow Immunization Card or a medical record from the physician. The record must be signed or stamped by the physician.
Polio
3 doses at any age
Diphtheria, Pertussis, and Tetanus (DPT)
4 doses at any age
Measles, Mumps, and Rubella (MMR)
Must have been given on or after 1st birthday*
1 dose
Hepatitis B
3 doses
Hib
1 dose
Varicella Chickenpox
1 dose or
Kindergarten and all grades 1-12 students
a physician-documented disease history
enrolling in a California school for the first time.
A four (4) day grace period is allowed if
vaccine was given 4 days prior to birthday.
All immunizations must be current, meaning the student has completed the above requirements or are not yet due for another dose. School Health Technicians and District Nurses are available to answer questions and provide assistance.
